Preparing for Your Visit

Registering at the Hospital
When you arrive for your stay or visit, you’ll be asked to complete some forms and answer questions about your medical history. Please bring your insurance card and a photo ID so that we can verify your coverage and identity.
Save Time, Pre-Register Online
You can pre-register for many of our hospital services from anywhere with an Internet connection, cutting back on the amount of paperwork you’ll need to complete in person. Simply choose your hospital, then follow the instructions and fill in the requested information. Upon arriving at the hospital admissions department, you’ll be asked to sign and complete a few additional forms.
Depending on the reason for your visit, your doctor may have other instructions you should follow to prepare for your visit.
Visiting Our Emergency Room? Check in Online
You can check in online for emergency care, so you can wait to be seen in the comfort of home, rather than the ER. Baptist Health System’s online self-scheduling lets you check in for estimated treatment times at any of our ERs.
