The Baptist Network for Cancer Care knows that the end of your treatment is not the end of your journey. Completing cancer treatment can be a difficult transition for many patients, that’s why we created a survivorship program just for you. This specialized clinic provides the resources you need to live the healthiest life after treatment possible. Survivorship offers information on wellness, support services, cancer surveillance and prevention, and can help you recognize and cope with any delayed side effects and late effects of treatment.

Your Survivorship Team

Our team provides the guidance you need to make the most of life after treatment and includes:

  • Cancer Care Nurse Navigator
  • Certified Cancer Coach
  • Resource Coordinator
  • Physical Therapist
  • Specialist Physician - medical oncologist, surgeon, radiation oncologist
  • Primary Care Physician

Your Transitional Consult and Personalized Plan

During your transitional consult, you will have the opportunity to discuss important survivorship issues such as screening and prevention guidelines, risks, diet, exercise and much more. In addition, your breast care nurse navigator will review your diagnosis history and treatment, including all your chemotherapy and radiation therapy.

You will receive customized recommendations for:

  • Medical surveillance - How to watch for cancer recurrence, secondary cancers and any late effects of treatment
  • Screening recommendations - What tests you need and how often
  • Healthcare providers to see
  • Wellness information tailored to your individual needs
  • Emotional and social support
  • Nutritional counseling
  • Exercise and fitness

You will leave your transitional consult with a written plan personalized to meet your needs and concerns. A copy will also be made available to your primary care physician and primary caregiver if you choose.
