As a patient, you have the right to inspect or review your medical records. Understanding your health records is key to making informed health decisions.
As a courtesy, your primary doctor will be notified of your request to inspect and/or review your medical record. Your doctor or a member of your health team must be present while you inspect or review your medical record. Please note that your medical
record is the property of the Baptist Health System. You may obtain a copy of your record, for a fee, after discharge. If you have questions about your medical record during your stay, please address those questions with your doctor and your nurse.
To receive a copy of your medical record after being discharged, you can complete the authorization form found below. Once you complete it accurately and legibly, you have several options for providing it to us:
