NP Chris Warmath will be leaving on December 31st, 2025. The physicians will be available to provide clinical services until mid-November and NP Warmath will be available until mid-December. Appointments can be scheduled by calling 210-598-5605.These providers will be transitioning to Christus Health at 250 W. Sunset, San Antonio, TX 78209. Your continued health and wellness are important to us, and we hope to make your transition to a new orthopedic provider as seamless as possible. We highly recommend that you identify another provider to assume your care as soon as possible.
