The Cardiovascular Destination Program at Baptist Health System connects patients with heart care and personalized coordination throughout the treatment journey. The program supports referrals, evaluation, treatment and recovery for patients traveling from across the region, nationally and internationally, while also providing coordination support for referring physicians.

Cardiovascular Care 

Patients in the Cardiovascular Destination Program have access to physicians experienced in diagnosing and treating complex cardiovascular conditions. The program’s services are delivered by multidisciplinary cardiovascular teams focused on evaluating and treating complex heart conditions. Available treatments may include: 

  • Complex coronary interventions, including complex percutaneous coronary intervention (PCI), to treat blocked heart arteries
  • Structural heart procedures such as transcatheter aortic valve replacement (TAVR), transcatheter mitral valve replacement (TMVR), left atrial appendage (LAA) closure, and coronary artery bypass grafting (CABG)
  • Heart failure care
  • Complex aortic and hybrid cardiovascular procedures
  • Cardiac imaging and diagnostic testing

Support for Patients 

Traveling for heart care can involve many steps. The Cardiovascular Destination Program provides personalized support to help coordinate care and simplify the experience. Each patient is connected with a cardiovascular nurse navigator who serves as a single point of contact throughout the process. Patient services include: 

  • Referral intake and eligibility review
  • Coordination of pre-arrival testing, imaging and consultations
  • Assistance arranging travel and lodging when needed
  • Guidance with insurance documentation and medical records
  • Coordination of procedure scheduling and hospital arrival
  • Support during recovery and discharge planning
  • Coordination of follow-up care with the patient’s local cardiologist

Support for Referring Physicians 

The program also provides support for physicians referring patients for cardiovascular care. Through a dedicated referral pathway, the program helps facilitate efficient evaluation of complex cases while maintaining communication with referring providers. Support for referring physicians includes: 

  • Rapid referral intake and clinical review
  • Direct communication with cardiovascular specialists
  • Updates throughout the patient’s evaluation and treatment
  • Coordinated care planning between specialists and referring physicians
  • Assistance transitioning patients back to their home cardiologist after treatment

Coordinated Care Journey 

The Cardiovascular Destination Program is designed to support patients through every stage of care. Key steps may include: 

  • Referral intake and eligibility review: The care team reviews the referral and medical history to determine appropriate evaluation.
  • Pre-arrival coordination: Imaging, telehealth consultations and travel arrangements may be coordinated prior to arrival.
  • Arrival and procedure planning:The care team helps streamline hospital arrival and procedure scheduling. 
  • Personalized recovery support: Patients receive coordinated recovery planning and discharge guidance.
  • Follow-up care coordination:The program works with the patient’s local cardiologist to support follow-up care after returning home.
